Please provide the following information for each of the reporting years 2022/23, 2023/24 and 2024/25:
- The total number of medication-related patient safety incidents reported within the Scottish Ambulance Service.
- The total number of Serious Incidents where a medication error or medication-related issue was identified as a contributing factor.
- The total number of medication-related patient safety incidents involving ambulance clinicians (e.g. Paramedics, Technicians or other frontline ambulance staff), where this information is available.
If possible, I would be grateful if the information could be provided as a yearly breakdown for each reporting year listed above.

The figures for questions 1 and 3 have been taken from medication-related incident records held by the Scottish Ambulance Service. These include incidents recorded under medication/drug-related categories, such as wrong medicine administered, incorrect dose or route, medicines administered outside relevant guidance, out-of-date medication, controlled drug-related incidents, and medicine supply or stock issues.
For question 3, the figures are the same as question 1, as the incidents identified relate to care or treatment delivered by ambulance clinicians or other relevant frontline/clinical ambulance staff, where this information is available from the incident record.
No Serious Incidents were identified for the requested period where a medication error or medication-related issue was recorded as a contributing factor.
